- Vertebral fracture
- Axial spondyloarthritis
- Paget's disease
- Spondylodiscitis
- Bone metastases
- Spondylolisthesis (pain aggravated by standing or walking)
- Scheuermann's disease
- DISH (diffuse idiopathic skeletal hyperostosis)
- Arachnoiditis (a rare cause resulting from nerve root sheath inflammation in the spinal canal)
